Passports
Passports may be applied for at the Ogemaw County Clerk’s office. A United States citizen needs a passport to enter the U.S. and most foreign countries.  Since the 9/11 incident, federal laws have changed and will continue to change to secure our national security.  That being said, it is advisable for U.S. citizens to give serious thought to acquiring a passport should travel outside the United States be a probability in the future.
 
APPLICATION PROCESS & REQUIREMENTS
 
If you have never had a passport, you must appear in person with a completed DSP-11 form available at the Ogemaw County Clerk’s office or from
The Passport Agency.
 
If you have never had a passport you will need to bring the following items:
 
1.       A certified copy of your birth record, i.e. it must have an embossed or raised seal on it
2.       Two passport quality photographs (2” x 2”) School or vending machine photos are not accepted.
3.       Your driver’s license or state identification card
4.       A check, money order or cash for payment
5.       All applicants over the age of 16 must execute their own application.

Passport Fees


  •  Age 16 and over (passport is valid for 10 years): $100.00

  • Under 16 years of age: $85.00
  • Renewal: $75.00
  • Expedite Service: $60.00
     
    Special Requirements for children under 14:
  • Both parents and/or child’s legal guardian(s) must appear with the child
  • If only one parent appears, you must submit the following:
           1.   Second parent’s written statement consenting to passport issuance for the child, must be notarized (form DS 3053)
           2.    Primary evidence of sole authority to apply OR a written statement (made under penalty of perjury) explaining the second parent’s unavailability. 

  • If the child has a previous passport, please bring it with you along with a certified copy of your child’s birth certificate.

Passport Renewal

Applications are available at the County Clerk’s office or Passport Agency.  You may renew your passport by mail if you meet the following criteria:
 
1.       Your previous passport must have been issued on or after your 16th birthday.
2.       Your passport must have been issued within the last 15 years.
3.       If your name has been changed due to marriage or by court order, you must submit proof of the name change.
4.       It is undamaged.
 
If you have had a passport, you will need:
 
1.       Your old passport (if you don’t have the expired passport, you will need to complete a DSP-64 form).
2.       Two passport pictures.
3.       Cash, check or money to cover the renewal fee of $75.00
 
The following forms can be accessed from the U.S. Passport Service Agency:



  • Form DS-11 = Application for a U.S. Passport

  • Form DS-82 = Application for Passport for Mail

  • Form DS-19 = Application for Amendment/Validation of Passport

  • Form DS-64 = Statement Regarding Lost or Stolen Passport
You can expect to receive your passport within approximately 6 weeks

Expedited Passport Service

There are instances whereby an individual may need to acquire a passport in a shortened time frame.  If that is the situation you may be facing, you may use an expedited service for processing your passport application.  This process may be initiated in the Ogemaw County Clerk’s office.  The fee is $60.00 for the expedite service.  Please contact the county clerk’s office for information regarding this service.


Credit/Debit Cards

The Ogemaw County Clerk’s Office now accepts payment by credit/debit card. To pay by credit/debit card


  • You must first contact the Clerk's Office, 989-345-0215

  • Request a pay location code and amount

  • Then you will be directed to call 888-604-7888 or log onto www.govpayexp.com   (This will direct you to the GPS website) to submit your payment.
